Mamas Connect Annual Waiver (2026)

Carefully review and complete this waiver before participating in any Mamas Connect programs, including Level Up Queens.

This waiver is required once per calendar year and is provided for general informational and consent purposes and does not constitute legal advice.

By participating in Mamas Connect Support Network Inc. programs, I acknowledge and agree to the following:

1. Scope of Participation

This waiver applies to all programs, groups, events, meetings, and activities offered by Mamas Connect Support Network Inc. during the calendar year, including virtual programs, in-person events, peer-support and accountability groups, digital platforms (including Zoom, WhatsApp, shared trackers), and any future programs introduced during the year.

This includes participation in Level Up Queens.

2. Nature of Programs

I understand that Mamas Connect programs, including Level Up Queens, are peer-support and accountability-based and are not therapy, counselling, coaching, or professional advisory services. No medical, mental health, legal, or financial advice is provided.

I understand that facilitators and organizers are not acting as licensed professionals and do not guarantee outcomes.

3. Personal Responsibility

I acknowledge that participation may involve personal reflection and discussion. I agree to take responsibility for my own wellbeing, boundaries, and participation choices, including seeking external support if needed.

4. Digital Participation & Visibility

I understand that participation may involve shared digital tools and group discussion, and that information I choose to share may be visible to other participants within the program.

5. Guests & Children

I understand that I may attend programs or events with guests, including children, family members, or friends.

I acknowledge that I am responsible for the supervision, safety, and conduct of any guests or children attending with me, and that their participation is voluntary and at my discretion. I confirm that I am the parent or legal guardian of any minor child for whom I am signing, or that I have authority to assume responsibility for the child(ren) in my care.

To the fullest extent permitted by law, I release and hold harmless Mamas Connect Support Network Inc., its founders, facilitators, volunteers, partners, and representatives (past, present, and future) from any claims or liabilities arising from my participation or the participation of my guests or children.

6. Assumption of Risk & Release

I voluntarily participate in Mamas Connect programs and assume all associated risks. To the fullest extent permitted by law, I release and hold harmless Mamas Connect Support Network Inc. and its representatives from any claims or legal actions arising from participation.

7. Governing Law

This agreement is governed by the laws of the Province of Ontario and the laws of Canada applicable therein.
